REQUIRED ATHLETIC FORMS - GO TO FORMS
According to FHSAA guidelines, all student-athletes MUST have all athletic forms on file and up to date to be eligible for participation. Please note: the physical form must be the FHSAA EL2 form, not the Broward County Health form. E-mail kimberlys@ccaeagles.org to check if your student's athletic forms are valid, all forms are valid for one full year from the date of the current physical.
